We are wanting to find out about the experience, both good and bad, people with neurodivergent conditions have in using mainstream services. Mainstream services are those which are open to anyone in the community. This will help services know what they need to do to let more people use them.

We expect the survey will take between 5 and 10 minutes to complete. However, if you complete the tell us more information sections, it may take considerably longer.

The first section is about who you are, including what neurodivergent condition(s) you have, where you live and how old you are.

The second section is about your experience of using services. This includes questions on using health services, leisure and community services. You are asked to rate your experiences between very poor and very good. You can then tell us more about your experience but don’t have to.

All the information given will be anonymous (we won’t know who you are)

You can answer as many or few questions as you want.
