We are wanting to find out about the experience of disabled people, when using GP (family doctor) services. We want to hear about your good and your bad experiences. We will use this information to make GP services more accessible for disabled people.
We expect the survey will take between 5 and 10 minutes to complete. However, if you complete the tell us more information sections, it may take longer.
The first section is about who you are, including what disabling condition(s) you have.
The second section is about your experience of using GP services. This includes questions on getting in and around the practice, being given information in a way which you understand and having reasonable adjustments made for you.
You are asked to rate your experiences between very poor and very good. You can then tell us more about your experience but don’t have to.
All the information given will be anonymous (we won’t know who you are)
You can answer as many or few questions as you want.
